Adult Day Health Care

Adult Day Health Care (ADHC) programs provide easily accessible services and care coordination to people living with HIV/AIDS at four of our facilities: West Village and Cylar House in Manhattan and East New York and the Women’s Health Center in Brooklyn.

All of our programs provide health care coordination and case management services to properly address the physical and mental health needs of our client.

Services provided daily include HIV/AIDS medication management, health care monitoring, case management, mental health and substance abuse services, life skills training, family support counseling, health education, vocational training, access to gymnasiums, hot meals, and transportation to ADHC and other health care services.

All of our ADHCs have extended weekday hours and are open Saturdays and Sundays. Two of our ADHCs also have residential options. East New York ADHC has 32 studio apartments and the Keith D. Cylar House ADHC has 32 studio apartments.

Eligibility Criteria

Adult Day Health Care

  • AIDS diagnosis or HIV symptomatic illness, currently or in the past
  • Active Medicaid or Medicaid eligible
  • Medical referral from Primary Care Provider
  • Medical documentation of HIV Status (T-Cell and Viral Load)
  • Proof of non-active TB (less than 3 months)
  • Chest X-Ray
  • Need for medication support & health care

Adult Day Health Care Residence

  • Enrolled and actively participating in the on site Adult Day Health Care program
  • Willingness to live in a supportive community
  • Demonstrates a need for co-located services
  • Must adhere to Code of Conduct.
