Program Director- Health Home

Overview

Program Director has the overall responsibility for administrative and clinical supervision of the program, and responsibility for ensuring the delivery of the six core Health Home service requirements. In addition the program director will assist with coordination of client services, staff recruitment, staff training, QA monthly summary reports, and development/implementation of program policies and procedures.

Responsibilities

  • Oversee client intake and enrollment process into Health Home
  • Ensure all Health Home tracking information has been submitted accurately and on time
  • Ensure health home clients are receiving integrated care appropriate for needs
  • Ensure the delivery of effective transitional and after care for clients leaving inpatient care
  • Use electronic health technology to monitor service delivery in program
  • Review client records to ensure that records are incompliance with internal and external standards
  • Participate in QA and CQI activities
  • Represent the Agency in a professional manner when interacting with community partners and providers; work cooperatively and network with partners and providers to build a comprehensive service network for clients
  • Consistently demonstrate a commitment to Housing Works’ core values of team work, open book management, advocacy and customer service
  • Work cooperatively with other HW departments to fully implement integrated care model for all clients

Qualifications

  • Masters Degree in Social Work, Psychology or other Human Services, plus two years supervisory experience
  • Ability to supervise, manager and provide leadership to staff members and trainees
  • Ability to communicate well, both written and oral
  • Excellent computer skills (Outlook, Word, Excel, databases)
  • Ability to work in a team management system
  • Understanding of chronic medical conditions, its impact on homelessness, substance use and mental illness
  • Knowledge of City, State, and Federal entitlement systems
